COVID-19 Screening Tool: All staff and parent/guardian of students should self screen every morning before coming on campus.  If you answer yes to any of the following please stay home, notify the school and if necessary call your health care provider.

Today or in the past 24 hours have you had any of the following:

• Feeling feverish or a measured temperature of 100.0 degrees Fahrenheit or greater
• Loss of taste or smell
• Cough
• Difficulty breathing or shortness of breath
• New or atypical headache
• Sore throat
• Significant or unexplained muscle pain or ache
• Diarrhea

If you test positive for COVID-19 or any other communicable disease, it is required to stay home until your symptoms have improved AND you have been fever free for 24 hours WITHOUT the use of medication.
